For EACH Medication, Provide:

  3. Frequency (e.g., "twice daily", "every morning")
  4. Route (by mouth, injection, patch, etc.)
  5. Prescribing doctor (which doctor prescribed it)
  6. What it treats (e.g., "for high blood pressure")

Include ALL of the following:
