eCoin® Tibial Nerve Stimulation for Overactive Bladder
The first and only implantable tibial nerve stimulator for OAB — a tiny coin-sized device placed near your ankle that delivers continuous nerve stimulation to reduce urgency, frequency, and leakage.
15-minute in-office procedure with local anesthesia. Educational content only.
What Is eCoin?
eCoin is a small, coin-shaped neurostimulator (about the size of a nickel) implanted just under the skin near your ankle. It delivers mild electrical pulses to the posterior tibial nerve — a major nerve that shares connections with the nerves controlling bladder function.
By stimulating this nerve continuously, eCoin "retrains" the bladder and reduces the signals causing urgency, frequency, and urge incontinence. Unlike external PTNS devices requiring weekly office visits or InterStim requiring a surgical implant in the lower back, eCoin offers a minimally invasive middle-ground solution.
How eCoin Works
The Science
The posterior tibial nerve originates from the same spinal segments (S2-S4) that control bladder function. Stimulating this nerve at the ankle sends signals back to the spinal cord and brain, modulating the bladder's overactive reflexes.
- Reduces involuntary bladder contractions.
- Decreases urgency signals from the bladder to the brain.
- Improves bladder capacity and reduces voiding frequency.
- Works continuously 24/7 without requiring external devices or recharging.
Device Features
- Size: Approximately 27mm diameter, 3mm thick (coin-sized).
- Battery Life: 3 years of continuous stimulation.
- Programmable: Stimulation parameters adjusted using external remote control.
- Implant Location: Just above the ankle, beneath the skin.
- Wireless: No external leads, patches, or charging required.
Who Is a Candidate?
eCoin is FDA-approved for adults with overactive bladder who have not responded adequately to behavioral therapy or medications. Ideal candidates include:
- Urgency urinary incontinence (leaking with strong urge).
- Urge-predominant mixed incontinence (urgency + some stress leakage).
- Bothersome urinary frequency (>8 voids per day) despite medication.
- Patients who cannot tolerate OAB medications due to side effects (dry mouth, constipation, memory issues).
- Desire for a reversible, minimally invasive option before considering InterStim or Botox.
Not ideal for: Patients with pure stress incontinence, urinary retention, neurologic conditions causing bladder dysfunction, or pacemaker/defibrillator incompatibility (consult with physician).
The Implantation Procedure
Before the Procedure
- Complete bladder symptom questionnaires (ICIQ, OAB-q).
- Void diary for 3 days to assess baseline symptoms.
- Stop blood thinners 5-7 days prior if directed.
- Procedure scheduled in office — no fasting or sedation required.
During the Procedure
- In-office procedure; takes about 15-20 minutes.
- Local anesthesia numbs the area near the ankle.
- Small incision (1-2 cm) made just above the medial ankle bone.
- eCoin device placed under the skin adjacent to the posterior tibial nerve.
- Incision closed with absorbable sutures; no external components visible.
- Device programmed and tested before you leave.
Recovery and Adjustment Period
Immediate Recovery
- Walk out of the office immediately after procedure.
- Mild soreness at implant site for 2-3 days (ibuprofen or acetaminophen as needed).
- Keep incision dry for 48 hours; then normal showering allowed.
- No heavy lifting or strenuous exercise for 1 week.
- Return to work next day for most patients.
Symptom Improvement Timeline
- Weeks 1-4: Device activated; you may feel mild tingling sensation near ankle (adjustable if uncomfortable).
- Weeks 6-12: Gradual reduction in urgency and frequency; keep symptom diary to track progress.
- 3 months: Peak benefit typically achieved; reassess with questionnaires and void diary.
- Follow-up visits at 1 week, 1 month, 3 months, and 6 months to optimize settings.
Clinical Outcomes
Published Evidence: The OASIS trial (pivotal FDA trial) demonstrated statistically significant improvements in urgency, frequency, and quality of life at 6 months with excellent safety profile.
eCoin vs. Other OAB Treatments
- vs. Medications: eCoin avoids side effects like dry mouth, constipation, cognitive changes. More effective for patients who don't respond to or can't tolerate medications.
- vs. Weekly PTNS (Urgent PC): eCoin provides continuous stimulation without weekly office visits. More convenient; similar efficacy.
- vs. InterStim (Sacral Neuromodulation): eCoin is less invasive (no surgery under anesthesia, no implant in lower back). Easier removal if needed. InterStim may offer slightly higher success rates but requires surgical trial and implant.
- vs. Botox Injections: eCoin is reversible and doesn't risk urinary retention. Botox requires repeat injections every 6-9 months and may require self-catheterization. eCoin offers continuous therapy.
Potential Risks and Considerations
Common Side Effects
- Mild pain or discomfort at implant site (usually resolves within 1 week).
- Tingling or buzzing sensation near ankle (can be adjusted or turned off).
- Temporary swelling or bruising at incision site.
Rare Complications
- Infection at implant site (<2%; treated with antibiotics or device removal if severe).
- Device migration or lead displacement (very rare; may require repositioning).
- Skin irritation or allergic reaction to implant materials (extremely rare).
- Non-responders: 25-35% of patients do not achieve adequate symptom relief; device can be removed.
Device Management
Remote Control Programming
You receive a small remote control to adjust stimulation settings at home:
- Increase or decrease stimulation intensity.
- Turn device on/off temporarily if needed.
- Check battery life (device alerts when nearing end of battery life after ~3 years).
Battery Replacement
- eCoin battery lasts approximately 3 years.
- Device is replaced with a simple in-office procedure similar to initial implantation.
- If symptoms have resolved, you may choose not to replace and have device removed.

Frequently Asked Questions
Will I feel the device under my skin?
The eCoin is very thin and sits flat under the skin. Most patients do not notice it after the incision heals. It does not interfere with walking, shoes, or normal activities.
Can I go through airport security or get an MRI?
Yes to airport security (device may set off metal detectors; carry your device ID card). MRI safety depends on the specific scanner; consult your physician and MRI technologist before imaging.
How long does it take to see results?
Most patients notice gradual improvement over 6-12 weeks. Peak benefit is typically achieved by 3 months. Some patients respond faster; others may take longer.
What if eCoin doesn't work for me?
The device can be easily removed in the office under local anesthesia. We then explore alternative treatments like InterStim, Botox, or pelvic floor therapy.
Is eCoin covered by insurance?
Coverage varies. Medicare and many commercial insurers cover eCoin when criteria are met. Our team verifies coverage and obtains prior authorization before scheduling.
Can I exercise with eCoin?
Yes. After the 1-week healing period, you can resume all normal activities including exercise, swimming, and sports. The device is water-resistant.
